Agents wanting to write business with First Benefits Insurance please contact Peg Pirrotta.
The appointment is simple. All you need is:
- A completed Agency Agreement (provided by FBI)
- A copy of the NC P&L License for each agent placing business with FBI
- Appointment of NC Agent form for each agent placing business with FBI
- A copy of the declaration page of your E&O policy
($1 million minimum required)
- A completed W-9 Form

- A copy of your corporate license (If incorporated)
