- Available for premiums over $1,000
- 25% Deposit Required (adjusted annually against prior year's audit and current payrolls) and held in escrow against termination
- Policyholder receives a monthly self-audit worksheet
- Payroll adjusted, premium calculated and payment submitted
- No installment fee
- Annual Audit
- Late fee of $25 for payments received after the installment due date
The premium payment is due by the 15th of the following month. If you are paying monthly and have not received your worksheet or it has been misplaced, call us at (800) 360-7867 and we will be happy to assist you.
MONTHLY - 25/10 PAYMENT PLAN
- Available for premiums over $1,000
- 25% down (January payment) with 10 equal payments (February - November)
- No security deposit required
- No installment fee
- Annual Audit
- Late fee of $25 for payments received after the installment due date
QUARTERLY PAYMENT PLAN
- Available for premiums over $1,000
- 25% (January payment) with 3 equal payments (April, July and October)
- No security deposit required
- No installment fee
- Annual Audit
- Late fee of $25 for payments received after the installment due date
ANNUAL PAYMENT PLAN
- Annualized Premiums of $1,000 or less require annual premium
(up-front payment)
- No security deposit required
- Annual Audit

- Completed Authorized Agreement Form
- Copy of voided check from insured's bank
